  • We do not accept health insurance. We are a membership based practice, and offer all our services for a low monthly cost. This is how we are able to provide a level of service and accessibility that is not found in a standard insurance based healthcare system.

  • No, DPC is not an insurance plan. It is recommended that you have health insurance for emergencies, injuries, specialists, and the possible need for surgery.

  • We require a minimum 3 month commitment when you sign up. After this there are no contracts and no long term commitments. We want you to be completely happy with our service. If we no longer meet your healthcare needs you can cancel at any time. There is a re-enrollment fee if you choose to reactivate your membership. 

  • Our goal at LIDM is to provide comprehensive, all access primary care throughout the year. While many patients in our practice may require infrequent in-office visits, receiving health care does not simply involve an in-person doctor's office visit once a year. Unhindered access via phone, text, or email, as well as telehealth visits are an essential part of addressing all health related concerns, both wellness and illness based. We want to continue to keep you healthy, and take care of you when you are sick. Easy access to your physician can result in fewer specialist, urgent care, or ER visits, saving you time and money.

  • Absolutely!For a one time fee we offer a comprehensive physical examination which includes a 2 month membership. If you decide you do not want to stay with us, you can cancel at the end of your trial.  

  • Yes, we offer same day appointments, both in person and via telehealth for all sick visits, chronic care management visits, and medication refills. We offer all other appointments within 1-2 weeks depending on your needs. 

  • We offer same day sick appointments and therefore do not offer walk in visits at this time.

  • We offer home visits for patients enrolled in our practice for an additional fee and are at Dr. Hersh’s discretion.  For homebound patients, please contact us to discuss membership options.

  • We draw your blood in office, and send the sample to Sherman Abrams lab. We include your insurance information with the blood work, and the payment for the lab will be billed to your insurance. Sherman Abrams accepts most insurances. If you are not sure they accept your insurance, we will find out for you, before sending out your sample. Sherman Abrams lab offers competitive pricing for self-pay as well.

  • You can call, text, email, or send us a message via our patient portal. There is no call center, a human answers the phone and responds to text messages during office hours.

  • In the event of a medical emergency we recommend you call 911 or go to your nearest hospital. In the event of urgent matters we are happy to help you at any hour. Call or text us for after hour responses. If you require non-urgent after hour assistance, please text us and we will respond as soon as we open the next day.

  • Some insurances require you to have a referral generated through their system in order to see a specialist. As I am not in-network with any insurances, I am therefore considered to be an out-of-network provider, and cannot generate these referrals. You can still join our practice, but would require an in-network physician to generate these referrals. If you are not sure if this applies to you, please call and we will be happy to find out for you.
